I feel it is fairly specific to my setting, but others could use it as an example for their context. It contains a registration with name, age, gender, and interests. You can hand this out at the first meeting before you divide out the groups or send it home to fill out and bring in later. The packet also gives reasons to meet in small groups and suggestions and tips when you are in a group that you don't like. It is generic enough to use for almost any church setting and even for adult Bible study.
