This is a gift inventory/time & talents form I created for my two-point parish in western Wisconsin. This has been adapted for smaller congregations to help congregations get a feel for the gifts members have to offer and are willing to offer. Submitted in both pdf and Publisher documents

This Gift Inventory is a great tool for ministry. It provides space for people to talk about their gifts in an non-threatening manner and is easily adaptable for use in any congregation. I especially like the family friendnly format. I could envision families sitting around the dinner table and discussing the spiritual gifts they have and the way they could use them in the church and in the kingdom of God. It is also open ended and creates room for suggestions and ideas. The most important thing about any form like this is that the information needs to be used. Once people show interest in using their gifts, they need to contacted right away! Many times people will fill out forms like this and never be asked to use the gifts they have indicated they have. That can lead to complacency when these tupes of forms are handed out. If your congregation and church staff intends to fully use this form to the best of their ability, it is a fantastic and simple way to survey a congregation's gifts.
